Pet Insurance Policy Conditions (U.S.)
I. Definitions Used in this Policy
Certain words and phrases in this policy are defined below. Defined words and phrases appear in boldface type and have the following meanings unless a different meaning is stated in the specific coverage or endorsement.
You, your
Named Insured named on the Declarations Page, and your spouse or partner if they reside at the same address.
We, Us, Our
the company providing this insurance, or the company’s designated agent.
Pet, Your Pet
The dog or cat listed on the Declarations Page for which premiums have been paid.
Behavioral Disorder
An abnormal, dysfunctional or unusual change in the temperament, activity or inactivity of a pet, without an underlying disease. Conduct problems incorporate, however are not restricted to, animosity, fear of abandonment, and fears.
Clinical Signs.
Changes in the pet’s normal health, physical functioning, or behavior as observed by an individual, documented in the pet’s medical records, or confirmed by a previous examination or treatment of the pet.
Copayments.
The percentage of the claim amount you are responsible for before any applicable deductible is applied.
Congenital Defect or Abnormality
A condition, abnormality or disability present at or from birth of your pet.
Curable medical condition
A medical condition or injury that is completely curable without recurrence or development of clinical signs.
Curable Exclusion Period
There is a 365-day exclusion period for any curable medical condition observed, documented, or confirmed on the policy start date, during the waiting period, or prior to 365 days before the pet’s first examination date. If the curable medical condition recurs within 365 days of the policy start date, a second 365-day deductible period will apply. If the same curable medical condition recurs during the second deductible period, coverage will be excluded for the lifetime of the pet.
Declaration Page
A document that forms part of the policy and specifies the Named Insured, Policy Number, Insured Pet, Insured Coverage Option selected, Period of Coverage, applicable co-payments and/or deductibles, and the maximum annual insurance coverage amount provided.
Deductible.
A fixed amount that you are responsible for for each period of coverage after co-payments are deducted and before claims settlement is received, and which will be deducted from any reimbursement to you.
Excess Insurance
Reimbursement under this policy is only available if you have exhausted the limits of coverage under other insurance.
Exclusions and Limitations
Circumstances, events or medical conditions not covered by this insurance.
Experimental Treatment
A drug, treatment, or procedure that has not been demonstrated, is limited to laboratory use, or has only been applied or tried on a limited number of pets, and is not widely recognized by the scientific community as a proven and effective treatment.
Initial Examination.
The earliest examination performed by a veterinarian after the effective date of coverage and during the policy period, if the pet has not been examined by a veterinarian within six months before the compelling date of inclusion.
Holistic Therapy
Treatment, including but not limited to acupuncture, aromatherapy, chiropractic and homeopathic therapies performed by a veterinarian to treat injury or disease.
Disease
Any change to a pet’s normal state of health, including illness, disease, or medical condition (excluding behavioral disorders), that is not the result of an accident.
Injury
Bodily harm caused to your pet as a result of normal activities or accidents.
Maximum Annual Coverage
The maximum amount we will cover during the policy period for each type of insured coverage covered by this policy.
Disease Condition
All clinical signs and symptoms arising from the same diagnostic classification or disease process.
Medically Necessary.
Any treatment or procedure directly and materially related to the covered illness or injury that is recommended by the treating veterinarian and documented in the pet’s medical records.
Original start date
If coverage is provided continuously without interruption, the effective date of the pet’s first period of coverage. If there is an interruption or gap in coverage, the date will be reset to the effective date of the next period of coverage following the interruption or gap in coverage.
Physical Therapy
Treatment, including but not limited to hydrotherapy, laser, underwater treadmill, kinesiotherapy, land exercises, massage, stretching, and electrical stimulation performed by a veterinarian to treat an injury or disease.
POLICY.
Means all the terms, conditions and endorsements (including the Declarations Page) of this policy.
Period of Insurance
The period of time from the effective date to the expiration date of the insurance as stated on the Declarations Page.
Pre-Existing Condition
A medical condition that first occurred or exhibited clinical signs prior to the effective date of this policy or that occurred or exhibited clinical signs during the waiting period of the policy or during the pet’s initial examination.
Preventive Care
Treatment, services, or procedures, such as physical examinations, medications, surgeries, vaccinations, and laboratory procedures, intended to prevent illness or injury or to promote general health, when no injury or illness has occurred.
Reasonable Expenses.
The charge normally charged by the treating veterinary hospital or by a licensed kennel or cattery for a given treatment, sickness visit, or procedure.
Sickness Relief Payments
The cost of veterinary care to treat pet injuries and illnesses.
Supplements.
Nutritional supplements, vitamins, herbal medicines, and other medications recommended by a veterinarian for the treatment of injury or illness that do not require a prescription.
Telemedicine
Treatment performed by a veterinarian to treat an injury or illness by telecommunications technology, including, but not limited to, telephone, e-mail, internet, text, and video.

Treatment
Veterinary care and prescription medications provided by or under the direct supervision of a veterinarian to treat an injury or illness of a pet within the United States or Canada.
Veterinarian
A licensed veterinarian residing and practicing within the United States or Canada from whom your pet was treated.
Waiting Period
For a period of fifteen (15) days from the effective date of this policy, you will not be covered for injury, illness, behavioral disorder, or any other coverage provided by this policy. This waiting period does not apply to renewal of coverage if renewal coverage is maintained on an ongoing basis. Any medical condition for which clinical symptoms are observed during the waiting period will be excluded from the policy as a pre-existing condition. (See also Section V.e.)
Working Dogs
A dog used for occupational, professional or business purposes.
II. insurance policy
When premiums are due and based on the statements you have made, we will provide coverage for your insured pet as specifically described in this policy and in accordance with the terms and conditions of this policy.
Except where stated to the contrary, all insured coverage is subject to all terms, conditions and limitations set forth herein and on the Declarations Page.

Insurance Coverage
Veterinary Expenses, Prescription Drugs and Supplements.
Coverage for the following expenses
a. the reasonable cost of medically necessary treatment by a veterinarian in the United States or Canada for your pet’s covered illness or injury during the policy period
b. the medically necessary and reasonable cost of medications available only by prescription from a veterinarian to treat a disease, injury, or behavioral disorder, for 90 days or less
c. the medically necessary and reasonable cost of supplements; c. the medically necessary and reasonable cost of dietary supplements; and d. the medically necessary and reasonable cost of any other medical supplies. The maximum amount we will pay for this coverage is up to the Annual Sum Insured shown on the Notice page of your policy.
Special Coverage and Limits
_D200D The following coverages do not increase the maximum annual insurance coverage amount shown on your policy’s Notice page, are not subject to co-payments, if any, and will not be considered in applying co-payments when the co-payment terms apply. Special coverage is not subject to any applicable deductibles.
Behavioral Therapy and Telemedicine
We will repay you for the accompanying costs1. reasonable expenses for office visits by a veterinarian to diagnose and treat behavioral disorders during the policy period
2. 2. reasonable medically necessary expenses for telehealth treatment provided by a veterinarian to support treatment of a covered injury, illness, or behavioral disorder.
The annual limit for this coverage is $1,000.
Advertising and Rewards
Coverage for reasonable and necessary advertising and reasonable rewards to be paid if your pet is stolen or lost during the policy period.
This coverage is limited to $1,000 per year.
If you discover that your pet is missing, you must immediately
1. notify the police and obtain a reference or case number and written confirmation of the report;
2. contact the nearest veterinary clinic and animal shelter (5 locations) where the pet was last seen
3. 3. complete and send us an invoice with all receipts for expenses incurred for the advertisement and reward payment.
Conditions Applicable to Advertising and Rewards
We will not reimburse you for the following
1. rewards that are not accompanied by a signed receipt with the name and address of the person who found the pet;
2. 2. any reward paid to a cohabitant, relative, employer, or acquaintance; or
3. any bounty resulting from the abandonment or willful concealment of a pet; and 4. any bounty resulting from the abandonment or willful concealment of a pet.
Boarding Fees.
During the period of coverage, we will reimburse you for the reasonable cost of boarding your pet at an approved kennel or cattery while you are hospitalized for your own illness, disease or physical injury.
The annual limit for this coverage is $1,000.
The following conditions must be met:
1. that your illness, disease or bodily injury has occurred or has been diagnosed and declared during the period of insurance
2. 2. to promptly submit a claim form completed by the attending physician and the kennel or cattery in the event of hospitalization due to your illness, disease or bodily injury; and
3. submit the original claim form from the kennel or cattery.
Conditions applicable to boarding fees
No repayment will be made in the accompanying cases1. if the hospital stay is less than 96 hours;
2. if the pet received treatment at a non-hospital care facility; or
3. 3. if the pet is hospitalized because of an illness, disease or physical injury that occurred or developed before the pet was covered by this policy;
4. hospitalization due to pregnancy or childbirth; or
5. 5. undergoing treatment unrelated to illness, disease, or physical injury
6. hospitalization for treatment of illness or injury due to alcoholism, drug addiction, attempted suicide, self-harm, or other harm 7.
7. loss due to theft or straying.
If your pet is stolen or lost and not found during the policy period, we will reimburse you for expenses paid for your pet.

This coverage is limited to $1,000 per year.
If you do not have original receipts to prove the amount paid for your pet, we will reimburse you for the local Humane Society adoption fee for the type of pet listed on the Claims Declaration page, or $150, whichever is less. As soon as you discover that your pet is missing, do the following
1. call the police and obtain a reference or case number and written confirmation of the report.
2. contact the five nearest veterinary clinics and animal shelters where the pet was last seen.
If the pet is not found within 30 days, send a completed claim form. Please enclose the original receipt for the pet’s payment.
If your pet is found or returned, you must reimburse us for all amounts paid by us under this policy within 90 days of the expiration of the period of coverage covered by this claim or within 90 days of the date your pet is returned.
Conditions Applicable to Damage Caused by Theft or Lost and Found
Compensation will not be provided in the following cases
1. you or your pet’s caretaker freely relinquishes your pet, even if deceived; 2. your pet is lost or stolen; 3. your pet is lost or stolen; or 4. your pet is stolen or lost for any reason.
Death from Injury or Disease
If your pet dies from injury or illness or is euthanized by a veterinarian during the policy period, we will reimburse you for the purchase price of your pet.
This coverage is limited to $1,000 per year.
If you do not have an original receipt to prove the purchase price of your pet, we will pay the local Humane Society adoption fee for the type of pet listed on the Declarations page, or $150, whichever is less.
Conditions applicable to death due to injury or illness
No reimbursement will be made if
1. the pet’s death is due to a pre-existing condition
2. the veterinarian is unable to confirm the death or sign the death claim form; or
3. if the pet was euthanized at the owner’s request and not at the veterinarian’s suggestion
4. if the death was caused by an illness of the pet over 6 years of age
5. if the euthanasia was caused by the pet’s behavioral disorder.
Vacation Cancellation
If your pet is injured or shows the first clinical signs of illness while traveling or at least 7 days prior to departure, your vacation must be canceled or shortened during the period of coverage and results in the need for immediate life-saving veterinary care, you will be covered for non-recoverable travel and lodging expenses. The annual limit for this coverage is $1,000.
Conditions Applicable to Vacation Cancellation
The following coverage is not provided
1. expenses related to vacations booked less than 28 days prior to the scheduled departure date.
2. expenses resulting from injury or illness that are not covered by the policy; 3. expenses for cancellation insurance
3. the cost of cancellation insurance
IV. co-payments and deductibles
You will be required to pay co-payments for compensable losses for which you receive medical treatment during the insurance period. In addition, a deductible (as shown on the Decarations Page) will be deducted from the insured amount.
Co-payments will be deducted from the total of all expenses for covered losses. Once the co-payment is applied, the deductible will be applied to the remaining amount. If the date of treatment for a covered loss spans more than one policy period, you will be responsible for the deductible amount for each policy period.
200D In addition to the application of co-payments and deductibles, there is a total limit of coverage for each policy period, which is listed on the notice page as the annual insurance coverage limit. (See also Section VI).
V. General Exclusions
This policy does not cover the following
Expenses incurred on behalf of your pet for matters not covered in Section III.
Expenses and payments in excess of the policy limits described in Section VI.a.
Expenses for treatment of illness or injury incurred outside the period of coverage while this insurance is not in effect.
Any portion of the cost of treatment for illness or injury in excess of the reasonable cost of treatment for such illness or injury.
Expenses for treatment or diagnostic tests for the following pre-existing conditions:
i. an Injury or Disease that occurs or first manifests clinical signs prior to the Effective Date of this Policy, an Illness that first manifests clinical signs during the waiting period beginning on the Effective Date of this Policy, or an Injury that occurs during the waiting period beginning on the Effective Date of this Policy; or
ii. an injury or illness with the same or identical diagnosis or clinical signs as an injury, illness or clinical signs suffered by the pet prior to the effective date of this policy, an illness with the same or identical diagnosis or clinical signs as an illness suffered by the pet during the waiting period beginning on the effective date of
this policy, an injury that occurs during the waiting period beginning on the effective date of this policy ii. an Injury that has the same or identical diagnosis or clinical manifestations as the Injury suffered by the Pet during the waiting period beginning on the effective date of the Policy; or
iii. an injury or illness arising out of, related to or attributable to an injury, illness or clinical signs suffered by the pet prior to the effective date of this policy, an illness arising out of, related to or attributable to an illness or clinical signs suffered by the pet during the waiting period beginning on the effective date of this policy,
or an illness arising out of, related to or attributable to an injury suffered by the pet during the waiting period beginning on the effective date of this policy injury arising out of, in connection with or resulting from an injury sustained by your pet during the waiting period commencing on the effective date of this policy. This exclusion applies regardless of where the injury, illness or clinical signs occur on the pet’s body. Pre-existing conditions do not include any Covered, ongoing medical condition that exhibits clinical signs after the original Effective Date and Waiting Period.
iv. Any curable medical condition observed, documented or confirmed up to 365 days prior to the initial policy date of this policy or during the waiting period of this policy shall be subject to a curable exclusion period of 365 days from the initial policy date of this policy. Any such curable medical condition that does not recur within the initial Curable Exclusion Period will be covered after the Curable Exclusion Period, provided an annual examination is performed by a veterinarian prior to the first recurrence of such curable medical condition.
If the Curable Medical Condition recurs within the Curable Exclusion Period after the initial start date, a second Curable Exclusion Period will apply after the first 365 days. At the end of the second Curable Exclusion Period, any such Curable Medical Condition that does not recur during the second Curable
Exclusion Period will be Covered after the second Curable Exclusion Period, provided that an annual veterinary examination is performed prior to the second recurrence of such Curable Medical Condition. If the same curable medical condition recurs during the Curable Exclusion Period, coverage will be excluded for the lifetime of the pet.
- Any behavioral disorder for which clinical symptoms were evident prior to the effective date of this policy or which becomes evident during the waiting period beginning on the effective date of this policy.
- Any congenital defect or abnormality for which clinical signs were evident prior to the effective date of the policy or which becomes apparent during the waiting period beginning on the effective date of the policy.
- Expenses arising out of or related to treatment related to cruciate ligament injury or rupture, patellar dislocation, or other soft tissue disorders of the knee for which clinical signs were evident during the first six months that this policy is in force. However, coverage will be provided even during the waiting period if your pet is seen by a veterinarian within 30 days of the inception date of the policy and the medical records indicate no pre-existing knee-related conditions.
- i. If your pet has been treated for or exhibited clinical signs of a cruciate or soft tissue injury to one knee prior to the effective date of this policy or during the first six months of this policy, the other knee is automatically excluded from coverage if no knee health certificate is provided as described in Section V. h. The other knee will be automatically excluded from coverage.
- Expenses arising out of or related to treatment related to hip dysplasia for which clinical symptoms occur during the first six months that this policy is in force.
- Disc Disease if another disc in the same or nearby spinal region (e.g., cervical, lumbosacral, etc.) has been previously treated or exhibited clinical symptoms prior to the effective date of this policy or during the waiting period beginning on the effective date.
- Expenses arising out of or related to treatment for oral hygiene, including but not limited to dental disease, malocclusion and deciduous teeth, where clinical signs (including but not limited to tartar, gingivitis, pulp exposure or halitosis) were observed prior to the effective date of this coverage or during the waiting period beginning on the effective date of this coverage.
- Food, including food prescribed by a veterinarian for the treatment or prevention of disease or injury.
- Expenses and payments for pets less than six (6) weeks of age.
- Illnesses that the pet contracted outside of the U.S. or Canada and that would not ordinarily have been contracted in the U.S. or Canada.
Expenses arising out of or related to
i. Breeding;
ii. pregnancy; or
iii. parturition or lactation; or
iv. treatment of offspring.
Reasonable expenses for medically necessary treatment of complications arising out of breeding, pregnancy, parturition or lactation will be reimbursed if the breeding date is after the waiting period of the effective date of this policy.
Bathing of Pets. You will be reimbursed for the cost of bathing your pet, unless your veterinarian has indicated that bathing is medically necessary and that only the veterinarian or veterinarian’s staff may bathe your pet.
Standard and preventive consideration, including yet not restricted to
i. vaccinations (and vaccine titers, nosodes)
ii. prophylaxis (including heartworm, flea and tick prevention); and
iii. Routine examinations
iv. dental prophylaxis
v. Treatment and therapy for weight loss.
The cost of admitting your pet to a veterinary hospital. Hospitalization is a covered expense only if medically necessary.
Cost of any form of housing, including rented or purchased cages.
Expenses for rented or purchased
i. swimming pools;
ii. hydrotherapy pools; and
iii. other pool or hydrotherapy equipment
iv. home-based physical therapy equipment, including but not limited to hydrotherapy, laser, underwater treadmill, and/or electrical stimulation
v. beds, orthopedic beds, and bedding.
The following treatment methods not performed by a veterinarian
i. Holistic therapy.
ii. physical therapy.
Experimental treatments or any treatment or procedure that does not meet accepted standards in veterinary medicine.
iii.Cloned pets or cloning procedures, whether or not considered experimental or research. iii.
Organ transplants that are not deemed medically necessary or not initially approved by us.
Expenses and installments emerging out of or connected withI. acquiescence or instructional courses, including pup classesii. training, corrective devices, or preventative products; and
iii. treatment of coprophagia or other dietary problems; oriv. Behavior disorder training.
iii. grooming, nail clippers, or grooming products; or iv.
Treatment or prevention of parasites or parasite-related conditions (internal or external). However, if no prophylaxis against parasites is available, this does not apply to
i. heart disease; or
ii. fleas; iii. ticks
iii. ticks; iv. roundworms
iv. roundworms;
v. tapeworm; or
vi. hookworm.
Selective or specialized treatments, including but not limited to
i. Tail docking;
ii. removal of dewclaws;
iii. removal of eyelashes
iv. Ear trimming
v. Castration or sterilization
vi. Cosmetic dentistry
vii. elective gastrectomy
viii. routine/preventive anal gland secretion.
viii. time and travel expenses to and from the veterinarian’s facility or hospital.
viii. expenses for illness or injury arising out of or related to the following
i. racing; or
ii. racing; and
iii. commercial security
iv. organized combat; and
v. any other occupational, professional or business use of your pet.
viii. costs and payments arising out of or related to the intentional injury or abuse (including persistent neglect) of your pet by you or a member of your family.
Costs and payments arising out of or related to an injury or illness for which you have been advised by a veterinarian to take action but have failed to follow the veterinarian’s recommendations.
House calls, unless urgent medical attention is required.
Expenses for euthanasia (unless recommended by the treating veterinarian), postmortem examination or testing, cremation, or other disposition of your pet. Disposal of pets deemed dangerous is not covered.
Costs or installments emerging out of or connected withi. encroachment; or
ii. war; or
iii. insurrection
iv. insurrection
v. Revolution, military force or usurped power
vi. Government seizure
vii. quarantine; or
viii. other measures related to public safety or health.
viii. treatment, death, or humanitarian destruction resulting from or related to avian influenza.
viii. any other general conditions set forth in Section VII, or any costs or payments set forth in Section VIII that are not in compliance with the conditions applicable to you.
Treatment against the advice of a veterinarian and any complications resulting therefrom, including those requested by you and provided by the veterinarian.
Amounts for failure to meet or comply with the conditions set forth in the “General Conditions”, “Pet Care” (VII.6) of this policy. If your pet has not been examined by a veterinarian within 6 months prior to the effective date of this policy, you must arrange for your pet to be examined at your own expense within 30 days of the effective date of this policy.
Any medical conditions, clinical symptoms, behavioral disorders, illnesses, and all related expenses observed or recorded at the time of the initial examination will automatically be excluded from coverage. In the event of an invalidation, the premium will be refunded.
In addition, coverage is also excluded for any medical condition, clinical sign, behavioral disorder or illness observed or recorded on the first examination performed on or after the effective date of this policy, and any condition arising out of or related thereto.
Expenses and payments arising out of or related to physical and/or holistic therapies to treat weight loss.
Expenses and payments related to shipping, processing, and/or handling.
VI. Limits of Insurance
Regardless of the number of claims made during the policy period, the total amount of insurance coverage for all covered expenses and payments during each policy period shall not exceed the amount stated under the maximum annual limit of insurance coverage.
All coverage under this policy shall terminate upon termination of the policy. See More…
